RENEW has provided Case Management services to people with developmental disAbilities in the five northeastern counties of Wyoming since 1972.
INDIVIDUALLY- SELECTED SERVICE COORDINATORS
ISC’s are people with the desire and purpose to help obtain needed supports and services.
RENEW’s ISCs are caring, committed, and talented people who meet current qualifications identified in Federal Standards for Qualified Mental Retardation Professionals (QMRP).
The responsibilities of RENEW’s ISCs are to:
- Provide services to enrich the personal lives of the people RENEW serves.
- Coordinate the team to develop the Individual Plan of Care (IPC)
- Understand the needs, desires, and preferences of those served.
- Facilitate assessments
- Advocate for those served at RENEW.
- Review and monitor service delivery
- Intervene in emergency situations (24 hour On-Call)
- Coordinate transition plans
- Be accessible for the convenience of those served
